Dear Valued Customer

The UK division of Smiths Medical International Limited is seeking to gather feedback on the service we have given our customers over the last 6 months.

The information that you provide will be used within the company to help us improve on our existing products and services. It will only be seen at an aggregated level by Smiths Medical employees (e.g. senior managers, marketing, sales, customer support and technical service staff). We will also send to everyone who responds to our survey a summary of the findings and the actions that we plan to take as a result.

Your individual responses and comments will NOT be passed on to anyone, unless you have specifically requested help with a problem or query, in which case your request will be sent to the relevant member of our team.
