1. Read the self-study guide. 
2. Complete the post-test and check your answers against the answer key provided. Refer back to the content for clarification of any incorrect answers. 
3. Complete the Evaluation/Registration Form at the back of this guide. Record the answers to the post-test.   
4. Complete the Educational Activity Evaluation Form at the back of the workbook.   
5. Submit the completed Educational Activity Evaluation Form and post-test to:

1. You will be sent your certificate of completion within six weeks. Records for educational activities will be maintained for five years.
