All material to be serviced or returned must have prior authorization from Smiths Medical PM, Inc., and a valid Return Authorization Number must appear on the shipping label, packing slip, purchase order, and any other related paperwork. Material received without such authorization will be refused at Smiths Medical PM, Inc.'s receiving dock and returned immediately to the customer.
When requesting authorization to return material, the following information must be provided: Customer purchase order number and date; quantity; Smiths Medical PM, Inc. product number; serial number if applicable; description of material to be returned; and reason for return. Upon receipt of authorized returned material an inspection of the material will be conducted by Smiths Medical PM, Inc. and appropriate action taken. Smiths Medical PM, Inc.'s decision regarding disposition of returned material is final.
All items to be returned shall be shipped prepaid to Smiths Medical PM, Inc. N7 W22025 Johnson Drive, Waukesha, WI 53186 unless otherwise advised by Smiths Medical PM, Inc.

Seller warrants to the original purchaser that the Product, including accessories, shall be free from defects in materials and workmanship under normal use, if used in accordance with its labeling for the period specified in the Operator’s Manual, the package inserts or other related documentation accompanying the Product.
